Pre-Authorization, Networks, and the No Surprises Act

A hospital can be in-network. The anesthesiologist working inside it may not be. That split, between the facility a family chooses and the clinicians who happen to staff it, is where surprise bills around a birth originate. Federal law now closes much of the gap. The No Surprises Act, effective since January 1, 2022, prohibits balance billing in the situations where families have the least choice. But pre-authorization requirements still gate what insurance covers, network verification still requires specific questions, and the rules differ for insured and uninsured families.
